Telehealth & Community Paramedicine

In St George, we're expanding healthcare access through a local telehealth and community paramedicine partnership.

In collaboration with the GRACE Innovation Center, St. George Community Paramedicine Program, and Jackson Memorial Library, we’re improving healthcare access by offering residents an integrated telehealth and community paramedicine program right here in St. George.

Telehealth

  1. Contact your medical provider and ask if your next appointment can be done through telehealth.
  2. Click on the blue button to contact StGCDC staff about reserving a telehealth device and space.

The telehealth program allows you to reserve a private space to meet remotely with medical providers such as your primary care physician or counselor. When you arrive for your telehealth appointment, you will be provided with an iPad and high speed internet access. A StGCDC staff member or volunteer will help you power up the iPad and login to your appointment.

NOTE: The St. George Community Development Corporation, GRACE Innovation Center, or Jackson Memorial Library are serving as community telehealth access points. They are not hospitals or urgent care sites and are not providing onsite medical treatment. We provide a private space and technology for residents to connect with licensed medical providers through secure telehealth visits. We do not diagnose, treat, or provide medical care on-site.

PRIVACY STATEMENT: Community telehealth access points do not store medical records. All medical records are maintained by your medical provider. The community telehealth access points may have video cameras in common areas, but not in rooms used for telehealth.

Community Paramedicine

  1. Contact your medical provider and ask if St. George Community Paramedics can assist with your care.
  2. Click on the button above to contact St. George EMS Chief Katie Jacob who will assist you with the Referral Form. Your medical provider must complete the referral form in order to receive Community Paramedicine (CP) services.

St. George EMS has operated a community paramedicine program since 2012. Through this program, EMS staff can provide some medical services to patients who have received a referral to the community paramedicine program.  Community Paramedics do not replace existing healthcare providers; instead, they work under a provider’s referral to deliver episodic, goal-oriented care that helps prevent unnecessary 911 calls and hospital visits.
